Featuring Ashley Sweaney, director of Ohio State’s Medication Management Program
According to the CDC, nearly 50% of adults in the United States have at least one chronic condition such as diabetes. This presentation will discuss how patients can utilize pharmacists to better understand their chronic conditions and improve their health. Topics covered include pharmacist training, top medication concerns, and Medication Therapy Management (MTM). MTM is a personalized service that achieves desired health outcomes by helping patients make the best use of their medications. Pharmacists are now providing these services in a variety of settings, including telephonic centers such as the Medication Management Program at The Ohio State University College of Pharmacy.
Ashley Sweaney is the director of the Medication Management Program, a part of the Institute of Therapeutic Innovations and Outcomes (ITIO–MMP), Ohio State College of Pharmacy. She currently oversees the call center and its staff. She has experience providing Medication Therapy Management both telephonically and within the community pharmacy setting.
